What you will do:<ul>
<li>Act as a Care Co-Ordinator to manage and complete day-to-day and planned interventions as allocated from the red zone.</li><li>Respond to requests that come via phone and email or as directed by senior team members to ensure continuity and quality of care.</li><li>Utilise your community experience, adept risk management, and excellent communication skills to deliver outstanding mental health support.</li><li>Collaborate effectively with team members and liaise with other healthcare professionals to ensure integrated patient care.</li><li>Contribute to the positive outcomes of the Early Intervention Psychosis team, supporting patients on their recovery journeys.</li></ul>
Requirements of the Community Psychiatric Nurse:<ul>
<li>Hold a Degree or equivalent in Mental Health Nursing.</li><li>Demonstrate experience working as a Mental Health Nurse in a community setting.</li><li>Maintain up-to-date NMC Registration.</li></ul>
