Exciting opportunities have arisen in our new Community Integrated Care Teams for Nurses, which delivers Prevention of Admission, Case Management and Core Nursing and Therapy.
Our team is the Welwyn & Hatfield Locality.
The HCT Integrated Community Care Teams are committed to delivering high quality, evidence- based nursing care to patients within their own homes and offer an integrated multidisciplinary approach in helping to support patient discharge and prevention of admission to hospital.
We offer a comprehensive induction programme and the opportunity to work in a friendly, dynamic, professional and caring team.
Successful candidates will need to have an ability to creatively problem solve and work on their own initiative and will need to demonstrate an understanding of current professional and community issues. Community nursing experience is preferred but there will be opportunities for training and development for the right applicants.
In addition to the basic salary, posts attract a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £1,000 and a maximum payment of £1,733 per annum (where applicable).
A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
