To provide safe and effective telephone advice, health assessment and information to the general public. To provide safe and effective health assessments and treatment of patients attending Primary Care Centres. To provide safe and effective health assessments and treatment of patients visited at home. To work as part of the multi-disciplinary team in providing quality, evidence-based health care to meet the immediate needs of the patient. To take an advanced role within the cross-disciplinary, multi-skilled team to ensure service quality in line with coporate goals and strategy and to assist in the development of new models for servicve delivery with have a positive impact on the health economy.
To receive telephone enquiries from patients, undertake an assessment of the presenting concerns and to give advice as appropriate using agreed protocols and referring to other service providers where necessary. To participate in the evaluation of the service that is offered and implement changes as required. To keep accurate and immediate records of every enquiry, including assessment and advice given regarding patient care, using the computerised call centre systems. Facilitate clear patient pathways through primary, community, secondary and tertiary care. Be responsible and accountable for assessing health and/or social needs of patients within the PCC setting, using agreed clincial protocols and clinical guidelines software. Hold a virtual case load and be accountable for delivery and integrated, quality patient care working autonomously. To hold nurse prescribing certificate and be competent to prescribe for patients whilst working autonomously.
Registered General Nurse Evidence of recent study (PREP) Degree level Nurse Practitioner Recognised Minor Illness. Injuries Cert. or willingness to undertake Recognised Nurse Prescribing Qualification Post basic certificate e.g. ENB course etc
Full / Part Time. Flexible hours working evenings, nights and weekend's. Hours of cover include public holidays. Standby sessions also required.
