Registered Nurse – Rehabilitation and Reablement in Southend Therapy and Recovery Team (START)
Annual leave: 23 days Per Year
Responsible to: Registered Manager
This service will require you to work alternate weekends
Responsibilities of the Role
-Provide professional, clinical input and advice to staff in the START service.
-Ensure you work within all current legislation, policies and procedures.
-Communicate effectively with patients and their families, ensuring accurate information is conveyed and documented.
-Assist in maintenance and monitoring of agreed quality standards in line with CQC.
-To ensure accurate data is contained within the relevant patient record systems within the areas managed.
-Assist in the development of clinical protocols ensuring these are reviewed / discussed with the Registered Manager.
-To act as medication ‘champion’ for the service – providing guidance and training to staff where needed.
-Carry out clinical file audits to ensure recording of patient care is accurate.
-Attend staff meetings and training, when required.
-Where necessary ensure that all disciplinary and grievance procedures are carried out in line with Company policy and statutory requirements.
-To liaise closely with other health professionals involved in the START service
-Work directly with patients and/or internal services and/or external agencies on day to day service issues.
-Primarily working at START head office and visiting patients in their own homes.
