Job Description

Our successful and established client is looking for a Community Nurse Manager in the South East or South West London area (based on post code), earning up to £45,000 per annum DOE. If so, this Community Nurse Manager role working for a well-established and progressive provider of complex care packages within the community, specialising in adult and paediatric care could be your next career move. You'll join during an exciting growth phase as the company has secured additional packages.

The successful Community Nurse Manager will be responsible for providing face to face support to adult clients and their families, whilst creating personalised care plans for each person. You will work closely with a core team of carers, training and supporting them with their clinical skills and activities.

If you were already working in this community nursing role, you'd have delivered the below:

? Designed and delivered a tailored care plan specific to an individual client's needs

? Effectively managed the MDT through strong communication and teamwork

? Provided ongoing monitoring, support and assessment of client's physical and physiological needs within your caseload

? Delivered nursing interventions including the management of medicine delivery

This Community Nurse role will be ideal for an RGN with a minimum of 6 months in the UK and an active NMC PIN; and one who has a supportive management style that can bring out the best in others. You'll have up to date experience of tracheostomy care and ventilators within the last two years and have advanced communication skills verbally and written to liaise effectively with patients, families and the MDT.
