This specialist 19 bedded unit offers comprehensive palliative care to a wide variety of patients with life-limiting illnesses. With the belief that their patients are entitled to live life to the fullest throughout their illness with dignity, empathy and respect - this unique London centre of excellence offers a very high standard of end-of-life nursing care.
You will review and triage all referrals to the Hospice and work with all staff to support and deliver safe, efficient and effective admission and discharge.
You will also act as a specialist resource for all health and social care professionals, patients and carers, liaising directly with referrers, and lead on complex discharges that require coordination of a number of services externally and internally.
Registered Nurse with full NMC registration.
A minimum of two years post-registration Palliative/End of life care experience.
Completion of a Mentorship qualification.
Be in possession of, or willing to undertake further post-graduate study in Palliative Care.
