As a community nurse, you will be providing high quality holistic care for people in their own homes, ranging from long term conditions, to complex and palliative care working under the direction of the District Nurse Team Leader. You'll be working on a rotational basis as part of a seven-day service covering from 8am to 8pm, with the support of agile technology to help you deliver timely, high quality care, with support from specialists across the Trust.
The successful candidate must demonstrate excellent clinical and communication skills to provide a high standard of care, and to ensure a positive patient experience. An enthusiasm for patient centered care, service development and multi-agency working is important. Experience in community care is an advantage however the post is also suitable for newly qualified due to our unique Preceptorship programme.
