Partnering Health Group are an established and expanding healthcare organisation responsible for providing a variety of urgent and primary health care services across Hampshire.
As an Nurse or Paramedic (Advanced Clinical Practitioner) (ANP or APP) at Partnering Health Limited (PHL) working as part of a team, In this role you will be responsible for the provision of safe and effective primary care appointments in line with GP Improved Access hub criteria, with full access to patient medical records. Our team of clinicians will provide appointments of an evening and weekend to local patients referred by their own GP surgery and 111. The post holder will take a pro-active approach to their own CPD, contribute to continually improving the clinical effectiveness of the clinics and work collaboratively and supportively with all colleagues. You ideally will know how to use EMIS clinical IT system but training will be provided.
The successful candidates will have a choice of shifts across the week and from a number of locations: Lymington, Ringwood and Winchester
Monday-Friday, with shift lengths varying from between 6.30pm to 11pm
Saturdays and Sundays and Bank Holidays 8am to 11pm.
Nurse or Paramedic (Advanced Clinical Practitioner) could also be an ideal opportunity as part of a portfolio role, with a local practice, A&E or Urgent Care service.
Nurse or Paramedic Key Responsibilities:
Assess undifferentiated patients with a range of acute, non-acute, chronic medical conditions, minor illness and minor injury. Take a patient history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans when working in a clinic or home visit environment.
Formulate a management plan for the patient. And/or to use other sources of healthcare professionals (such as GP or Pharmacists) to if required to support diagnosis or next course of action
Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary and tertiary care
Prescribe medication as required within your clinical competence.
Recognise the need and initiate emergency treatment in order to save life or to safeguard the interests of the patient and refer to emergency services when appropriate
To ensure that patients are safely discharged from our care with appropriate instructions for after care and follow up
To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence
Maintain and develop clinical competence and knowledge of care delivery within primary care and urgent care setting using both formal and informal training methods.
To work within the professional Code of Conduct and other directives as defined by the NMC/HCPC
To undertake appropriate personal and professional development to meet the requirements of NMC/HCPC Registration.
Ensure effective use of Clinical IT system, utilisation of templates, coding to ensure delivery of QOF, LCS, CQUINs, DES where appropriate
Nurse or Paramedic Skills & Experience:
Qualified non medical prescriber.
Registered with NMC / HCPC
Experience working in a primary care planned care clinics and same day/urgent care clinics
Experience of Chronic Disease Management
At least three years post qualified
You must be competent at taking a clinical history excluding red flags and undertake a physical examination of the patient for patients presenting with undifferentiated conditions
Good clinical skills in assessment, diagnosis and management of acute Primary care presentations of minor illness and injury
Be able to clear an enhanced DBS check/ Must be authorised to work in the EEC
Urgent care setting
Radiology and Interpretation course
Experience of working in a multi-disciplinary team
Familiar with Internet Healthcare resources
We provide an attractive rate of pay dependent upon skills and experience and enhanced rates for unsocial hours. Ranging from £25 – £35 per hour.
