B1 - Acute Frailty Ward

We are therefore looking to recruit motivated, hardworking nurses who have excellent communication skills & a passion for their role and improving patient care When working clinically we expect you to perform with passion and drive, provide clinically excellent care, and be supportive to patients, relatives, and other MDT members. As a registered nurse you will be expected to promote and lead the delivery of an excellent standard of patient centred care in a supported environment.
There are exciting opportunities (Full and Part Time) to join our highly motivated, dynamic and hardworking team within elderly medicine. We are looking for nurses with a minimum of 12-months post registration experience, ideally with a passion for frailty and a background in complex medicine.
Ward B1 is a 23 bedded male and female ward elderly care ward supported by 2 excellent consultants and a ward-based therapy team.
We provide care for acutely unwell elderly patients and support them through to discharge. The role provides opportunity for experience of care of the acutely unwell patient. The role also includes support for the complex discharge process. This involves working with the multidisciplinary team and attending multidisciplinary meetings around discharge, best interest meetings and continuing health care meetings with the safeguarding team and social workers. We also care for patients awaiting Intermediate care and are require more rehabilitation or an increased care package
There are opportunities to gain experience around the discharge process, dementia care and rehabilitation. You will also learn more about the continuing health care process and the discharge process as a whole, you will work with occupational therapists, physiotherapists, medical staff and a discharge planning nurse to ensure your patients receive the best care.
