Job Description

The aim of the service is to provide community based care and rehabilitation to prevent unnecessary hospital admission, facilitate hospital discharge, and promote independence and reduce reliance on long term social care. The successful applicants will work alongside Physiotherapists, Occupational Therapists, Social Workers, Social Care Assessors, Pharmacists, and Rehabilitation Support Workers as part of the multi-disciplinary team.

The successful applicant will be responsible, in conjunction with the Clinical Nurse Specialist for the development of the nursing role and nursing teams within the service. The nursing role includes providing assessment of and intervention planning for pressure care, continence, medication review / reconciliation / administration, urgent wound management, and rehabilitation. The nursing team plays a key role in the prevention of unnecessary admission to hospital.

These roles would be ideal for Band 7 or experienced Band 6 Nurses looking to develop in the specialist areas of community rehabilitation, admission avoidance, holistic assessment, and integrated care.

The successful applicant will be primarily aligned to the London Borough of Lambeth but will be expected to work closely with colleagues in Southwark to support the provision of Guy's and St. Thomas' community services.

Candidates who are asked to attend interview will be asked to complete a 10 minute presentation on the Integration is high on the agenda in health & social care teams. What is your understanding of integration as a concept in practice and how do you think you can influence this in your role as a band 7 lead in intermediate care Lambeth?
