Job Description

Our service users have varied health and social care needs where you will get to use a range of interventions within a supportive multidisciplinary Team with service users on a number of care pathways across North Manchester.
We are working towards covering a 7 day a week service and you will be asked to work a minimal number of weekend shifts over the year on a rota basis.
We work from Harpurhey District office (soon to be co-located with the North west team at Macartney house) in the centre of North Manchester catchment area.
You will be a nurse acting as care coordinator who will be responsible for the management of a mixed caseload of clients with complex mental health needs and associated social care issues, for which risk assessment and risk management are essential requirements. You will work with other members of the team to ensure service users receive individualised recovery focused care plans to ensure they receive the most appropriate care to best meet their needs. There will be an expectation that you support the duty rota system and assist in responding to any potential crisis or safeguarding work that comes into the team.
