This post provides a challenging opportunity to work within an established fully integrated and supportive team.
You will hold and manage a caseload of service users who require care coordination under the Care Programme Approach. By assessing your clients and carers needs you will be able to access and sign post to a wide range of community resources and provider services available in Trafford.
The post holder will be expected to participate in providing quality first line psychosocial assessments as part of GMMH's Functional CMHT's duty role and work proactively in forging professional working relationships with multi-agencies. There is an expectation to work weekends /BH's as part of GMMH's Acute Care Pathway provision of care on a rota basis.
You will receive regular clinical and managerial supervision and developmental appraisal thus enhancing your current practice and future career prospects.
