Community Nursing has three inpatient units- Minnie Kidd House, Pulross, Amputee rehabilitation unit.

Staff Nurse Band 5 - Community inpatient Services
We have vacancy in Pulross and Minnie Kidd House, you can apply and specify which unit you would like to be interviewed for or discuss at interview and decide where you would like to go. If you wish to join Amputee Rehab Unit you may be interviewed for this area and placed on a waiting list for when a vacancy may arise

Now is a fantastic time to join Community Nursing, we are currently implementing lots of exciting changes and new roles. There are more initiatives for patients to rehabilitate for earlier discharge.

Minnie Kidd House

A 28 bedded specialist unit offering a homely, caring environment. We provide specialized nursing care for patients with complex needs such as Tracheostomy care, neurology, diabetic management and end of life care. We are based in Lambeth. This role provides an opportunity for a band 5 nurse to develop leadership skills and team work.

We care for complex, intense and unpredictable medical and nursing needs over a 24 hour period. The team is supported by 2 Consultant Physicians and excellent medicines management support by the Pharmacist, with input from other specialist services. Pulross

The Pulross Intermediate Care Centre is located in central Brixton. This is an award winning building which has 20 beds and a robust multidisciplinary team led by nurses.

The unit comprises of 14 intermediate rehabilitation bed where patients admitted must have clear identifiable Rehabilitation goals. The primary function of Pulross are the following:

• Avoid admissions to hospitals
• Rehabilitation post acute admission
• Prevention into early admission into long term care
In September 2016 the unit opened 6 Level 2b neuro rehabiliation beds which is part of the KHP South East London Neuro Rehabilitation Service ( SELNRS ). This is an exciting opportunity to work as part of an excellent MDT in the field of neurorehabilitation.

The Amputee Rehabilitation Unit (ARU)
A twelve bedded inpatient unit based at the Lambeth Care Centre in Kennington.
Patients are admitted for a six week rehab stay from London and the South East for intensive amputee rehabilitation. All patients will have at least one major amputation. The case load is mixed and the range of reasons for amputation includes Vascular Disease and Diabetes, Trauma, Sepsis, Oncology and others. The age range of our patients ranges from 18 years and over. The caseload is therefore challenging but extremely rewarding and allows for variety on a daily basis.
The ARU provides specialist care and rehabilitation for amputee patients with varying levels of functional ability.
Our patients will stay at the ARU and receive therapy every day in our indoor gym and also use our fantastic landscaped gardens outside along with community access visits.
The ARU promotes independence and patients are challenged daily to meet their goals designed by the Multi Disciplinary Team. We provide an enabling care approach and work closely with the MDT.
We have strong links to Guy's and St Thomas' acute services and provide high standards of specialist care including;
Complex wound management including Vac Therapy and debridement
Diabetes specialist care
Falls management
Counselling and psychological support
Our medical team provide support three days a week but we are a nurse led ward, working closely with our amputee rehabilitation clinical lead.
We have our own Personal Discharge Facilitator who prides herself on providing an effective safe discharge plan at all times and allows the nursing team to focus on delivering care.
Nursing staff receive bespoke in-house rehabilitation training from our clinical lead, nursing leads, physiotherapists, occupational therapists and tissue viability nurse.
Our Nursing staff have a great commitment to our patients and strive to be the best at what we do.
We provide excellent support to our nursing students and welcome newly qualified staff,
