The opportunity has arisen for an enthusiastic Registered Mental Nealth nurse to join our friendly multi-disciplinary Community Mental Health Team.
This post provides a challenging opportunity to work within an established fully integrated supportive Community Mental Health Team.
You will hold and manage a caseload of people and have Care Coordinating responsibilities under the Care Programme Approach Guidelines. By assessing your clients and carers needs you will be able to access and sign post to a wide range of community resources and provider services available in Salford. The post holder will be expected to participate in providing quality first line psychosocial assessments as part of GMMH's Functional CMHT's duty role and work proactively in forging professional working relationships with multi-agencies. There is an expectation to work weekends /BH's as part of GMMH's Acute Care Pathway provision of care. This is on a rota basis between the four community teams.
