What you will do:

- Conduct independent assessments, treatment planning, and discharge planning for patients in the community rehab team.

- Engage in equipment reasoning and ordering to optimize patient care and outcomes.

- Utilize recognisable outcome measures to assess patient progress and modify treatment strategies accordingly.

- Lead family meetings to ensure cohesive care plans and support for patients and their families.

- Drive between various community settings to deliver care, as being a car driver is preferable for this community-focused role.

Requirements:

- Registered with the HCPC.

- Proven experience working in the NHS and as a qualified Occupational Therapist, preferably within community and rehab settings.

- A valid driving license as driving is beneficial for this role.
