This is an exciting opportunity for a Band 6 Physiotherapist to join our Discharge to Assess Team based in Basildon, UK. Join us as a highly motivated, enthusiastic, and experienced Band 6 Senior Physiotherapist in our Discharge to Assess Service, within our Clinical Support Service Division. Collaborate with a Senior Physiotherapist, CHC/AACC Nurses, Domiciliary Care staff, and care home staff to ensure effective and efficient patient discharges. To thrive in this role, you must be HCPC registered and ideally, a car driver to access various locations as required.

What you will do:

- Conduct assessments and create individualised treatment plans tailored to each patient's needs.

- Work closely with multidisciplinary teams to facilitate effective patient discharge processes.

- Provide expert physiotherapy advice to support patient rehabilitation and recovery.

- Monitor and assess patient progress, adjusting treatment plans as necessary.

- Liaise with external care providers and stakeholders to ensure seamless care transitions.

Candidates must be HCPC-registered and possess a valid driving licence.
