Experienced Occupational Therapist - Reablement Team, Trowbridge (UK) - £36.54 Hourly

Join us for a rewarding opportunity as an Experienced Occupational Therapist within the Reablement Team in Trowbridge. This role offers a competitive hourly rate of £36.54, with full-time hours from 09:00 to 17:00. It is a perfect chance for an experienced OT looking to make a difference in the lives of our clients. We're looking for a qualified OT with at least two years of post-qualification experience. Holding a valid full UK driver's license is required, as they'll be conducting visits to clients in their own homes or other settings, such as hospitals.

What you will do:

* Conduct comprehensive strengths-based assessments for clients after illness or injury

* Collaborate with clients, their families, and multidisciplinary teams to design personalised reablement plans

* Visit clients in their homes and community settings to promote independence and well-being

* Monitor client progress and adjust reablement strategies as required to ensure successful outcomes

* Document and report on client progress, liaising with other health care professionals as needed
