What you will do:

* Conduct detailed mental health assessments for forensic clients in various secure settings including high, medium, and low security units, as well as prisons.

* Develop and manage individual care plans, ensuring they are tailored to each service user's unique needs.

* Coordinate and facilitate the discharge of patients from out-of-area placements back into community settings.

* Liaise consistently with service users, carers, and a wide array of health and criminal justice professionals to provide comprehensive care.

* Assess and manage risks, employing evidence-based psychosocial interventions to support recovery and rehabilitation.

* Maintain meticulous records, completing necessary documentation such as facility reports, panel applications, and referral forms.

* Work collaboratively within a multidisciplinary team, contributing to the development and enhancement of the forensic care pathway.
