Your Role:

--Provide evidence-based care for acute and chronic conditions

--Support health promotion, screening, and patient education

--Collaborate with a multidisciplinary team to improve patient outcomes

What We Offer:

--Competitive salary up to £45,000 p/annum pro rata (negotiable based on experience)

--Part-Time hours up to 30 hours p/week

Requirements:

--Registered Nurse with NMC registration

--Experience in chronic disease management (Diabetes or Asthma)

--Excellent communication and interpersonal skills

--IT proficiency and attention to detail
