Job overview
We are pleased to advertise a 6 month secondment opportunity for an Independent Prescriber in the West Kent Primary Care Mental Health Team at Highlands House in Tunbridge Wells & Albion Place in Maidstone.
We are looking for a qualified Independent Prescriber who is a mental health nurse or pharmacist, ideally with specialist knowledge, however applications from newly qualified prescribers with substantial mental health experience will be considered.
The role will be an integral part of the multidisciplinary team which comprises of Nurses, trainee ACPs, RGNs, and Admin. The Independent Prescriber will work within a defined scope of practice and to agreed clinical standards applied within the local setting.
The clinician would be expected to provide clinical assessment , formulation and treatment, including the prescribing of medications. The role will focus on working in the GP practices working alongside the GPs and trainee ACPs and supervised by Consultant Psychiatrists, reviewing and adjusting medication as part of the prescribing practice through the use of the clinical management plan. The clinician would be expected to provide advice to GPs, patients and other professionals on the use of mental health medications. The role will improve the quality of and access to health care provision and strengthen professional leadership, supporting the development and practice of junior clinicians.
Main duties of the job
Key elements of the role will be:
Work as an autonomous prescriber utilising medication and diagnostic skills to provide and prescribe care and treatment
Support education and development of other staff in medicines management and evidence-based use of medicines
Perform medication reviews for patients attending the GP practices or KMPT clinics across the locality with a minimum of 20 appointments each week
To be the lead HCP for a maximum of 40 clients (on standard care pathway only)
Undertake a full holistic assessment (with a focus on physical health improvement), liaising and referring to other agencies and professionals as necessary. Undertake reviews of medicines and promote patient choice and effective outcomes
Develop and use diagnostic skills, initiate investigation, prescribing care and treatment as appropriate
Provide expert clinical advice and work collaboratively with other health professionals and agencies within the Directorate
Independently prescribe medication within a defined scope of practice
