Job Description
Easington South Community Mental Health team are looking to expand our current workforce.It is an exciting time to join the team as we work together to embed a newly created pathway with a strong focus on patient centred trauma informed care.
We are actively looking for a Band 6, Senior Community Mental Health Practitioner to join our MDT.
Main duties of the job
Senior Practitioners will need a good understanding of collaborative practice working with adults of varying ages and complex needs, including service users, their carers' and families.
As a core member of the team, you will also be responsible for:-
- your own caseload
- providing assessments
- care co-ordination
- delivering intervention in the community.
- identified and central to their CPA Well-being and Recovery plans
- promoting social inclusion
- independence and well-being
- excellent assessment skills
- interpersonal and team working skills
- be able to work collaboratively with a range of statutory and third sector agencies.
Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010. A lease car scheme is in operation as well as other NHS staff benefits.
