Job Description

Detailed job description and main responsibilities
Support the School Nursing Service to establish a wider relational team around the 2 Dixons schools of which the Family Support Worker will be part. It is envisaged that the wider relational team will include members of the district wide School Nursing Service as well as other key colleagues from the school setting as well as other partners.

Work with the School Nursing Team Leader and Core 0-19 Service Manager to introduce the School Nursing Service to the 2 Dixons Schools, implement and embed the Family Support Worker role and map out and deliver the School Nursing offer to the 2 Dixons Schools for the Academic Per Year September 22 - July 23. The allocated School Nurse (SCPHN) will lead on the delivery of the Healthy Child Programme. The Family Support worker will support this delivery with other members of the School Nursing service.

The Family Support Worker will receive leadership, governance, training and clinical and safeguarding supervision and support from BDCFT Community Children's Services. They will be a key member of the District Wide School Nursing Service and will have a base at Dixons Trinity Academy as well, where they will be accessible and visible in School for the wider Dixons Team, the children, young people, and their parents / carers.

Work alongside other key agencies to identify and support children, young people and families such as learning mentors, pastoral support workers, health visitors, social workers promoting the health needs of all students.

Represent BDCFT and the District Wide School Nursing service in the 2 Dixons Schools forming positive working relationships that add value and benefit to the wider school team and student community.

Work closely with the Dixons Senior Team and the Core 0-19 Service Manager on the evaluation of this project, recording and collating evidence of outputs throughout and evaluating via the Dixons Research School. This will include monitoring and recording all contacts, interventions and pieces of work offered and completed during the academic Per Year.

Develop excellent working relationships with the community and parents, managing and promoting engagement in learning as well as understanding and supporting children's development and health needs at place.

Provide and receive complex or sensitive information, communicating with reassurance, empathy and compassion to engage with the child / young person and their families, especially where there are barriers to understanding i.e., language, learning disability

Where necessary and in addition to the school Nurse offer and Healthy Child Programme, work to connect children, young people and their families to the local Family Hub.

Implement care packages under the supervision of the Band 6 School Nurse (SCPHN) in a range of settings, whether that be in school, the local Family Hub or at the family home. Ensure that the health needs of the child remain at the fore and that interventions are linked to school assessments / profiles.

Use your professional, analytical and judgement skills to support your assessment of health needs and act as an advocate and conduit to ensure that the voice of the child / young person / family are heard and that they are connected to the right health professional, right health service at the right time.

Plan and organise your own workload which could include joint home visits with school colleagues to family homes, transition and new starter support work and activities, representation at parent evenings.

Person specification

Qualifications
Essential criteria
  • Minimum level 2 Maths and English
  • Current professional experience and qualification as a community nursery nurse or Family Support Worker with: CACHE Level 3, Diploma in Childcare and Education (DCE) (Previously known as NNEB) or NVQ Level 3 in Childcare and Education
  • Experience of working in a Community Children's Service or in a school-based service.


Desirable criteria
  • Evidence of leading and completing project work


Experience
Essential criteria
  • Experience of working in a childcare, social care, school setting
  • Experience of delivering direct clinical work within a community children's service.
  • Experience of working within a multidisciplinary team.
  • Experience, knowledge and awareness of the potential health needs of children & young people
  • Experience of coordinating or delivering health and wellbeing activities, interventions, assessments.

  • Experience of involvement in leadership, service improvement, quality and governance, research and audit and gaining service user feedb
