Job Description

Detailed job description and main responsibilities
3. Main duties:
To work as a Senior Practitioner ensuring that the assessment, planning,
implementation and evaluation of care for a designated client group is carried out to the highest standard.
To manage a caseload and delegate appropriately to clinical practitioners and assistant practitioners.
Assists the Team Manager and Assistant Ward Managers in ensuring that agreed standards of professional activities within the unit are achieved.
Take a lead role in quality monitoring the service and delivery of care.
Formulates and influences new ways of working within the service. Supporting planning for discharge upon admission and working with the wider clinical network that supports this process.
Support the Team Manager and Assistant Ward Managers in the staffing co-ordination/deployment on the shift.
Assist the Ward Manager and Assistant Ward Managers in ensuring that effective communications are maintained with other professionals, disciplines and teams.
Guide/mentor/supervise supporting staff as required facilitate/deliver ongoing or functionally relevant activities in the most appropriate setting.
Evaluate treatment outcomes and meet emerging needs. Referring on to address unmet needs as appropriate and necessary.
Acts as a role model to staff; demonstrating compassionate dementia care whilst sharing your own dementia knowledge.
Acts as a consultant to staff on clinical and operational matters; providing advice on clinical and professional matters. Working closely with the Community Matron for Physical Health.
Ensure that patient records are maintained and that information is passed on in a co-ordinated manner.
Provide training or mentorship to students; preceptorship nurses.
To participate in health studies and research into new patterns/methods of care to further enhance set standards of care. Supported by senior members of the clinical team.
Ensures that Trust Policies, Protocols, Procedures and Guidelines and Professional Codes of Conducts are implemented and maintained. Suggest any policy revisions to the Team Manager.
Undertakes in house and external auditing, monitoring quality and maintaining high standards expected.

4. Working as part of a Team
Provides clinical and professional leadership to a team.
Gives professional advice and clinical support to staff within the service and refers to the Team Manager or other professionals when necessary.
In conjunction with the Team Manager and Assistant Ward Managers identify staff development needs.
5. Managing Self
Participate in regular supervision.
Attend all mandatory training.
Participate annually identifying, developing and agreeing your own development plan with your line manager using the Trust appraisal and KSF process.
Comply with all Trust policies, procedures and protocols.
Pay regard to materials and equipment.
Carry out duties with due regard to the Trust's Equal Opportunity Policy.
Seek advice and support from Team Leader whenever necessary.

6. Staff Supervision and Support
Provides training and mentorship to students and preceptorship nurses.
Supports the maintenance of good working relationships across different professions, disciplines and teams.

7. Financial Responsibility
Assists the Ward Manager and Assistant Ward Managers in ensuring the economical and effective use of equipment/supplies.
Informs the Ward Manager of the needs of the Department as regards
supplies/equipment as requested.

The Governments' proposal to mandate Covid 19 vaccines for all health care workers is currently under review. Until there is a formal directive in place regarding mandatory vaccines as a condition of your employment, Bradford District Care Trust does not require you to be fully vaccinated as part of your employment conditions.
