Job Description

Detailed job description and main responsibilities
  • The post holder is expected to work as part of the district nursing team, under indirect supervision of the district nurse. The post holder is accountable for own professional actions in line with the NMC guidance.
  • Assess individuals and population needs which affect their health and
    social well-being and reporting back to the district nurse caseload holder as appropriate.
  • Plan, implement, monitor, and review programmes of health interventions for individuals and populations, which are to be delivered with other colleagues and agencies. Liaise closely with the district nurse to discuss changes in nursing care e.g., medication reviews, care plans.
  • Assess risk that may affect patients or care providers.
  • To work collaboratively with general practice colleagues and other primary health care team members.
  • Promote people's equality, diversity, and rights.
  • Enable people to manage disability, loss, and change.
  • Contribute to the protection of individuals from abuse.
  • Administer immunisations and vaccinations in accordance with national and local programmes.
  • Work within relevant legislation, for example, health and safety, NMC code of professional conduct.
  • Work within all relevant BDCFT policies and procedural guidelines.
  • Undertake a variety of technical nursing treatments for a range of patients with different health needs. i.e., complex wound-management skills, bowel care.
  • Administer prescribed medication via syringe drivers and participate with Gold Standards Framework/Liverpool Care Pathways.
  • Enable/encourage patients and carers to develop their knowledge and understanding of their own health and well-being enabling self-management of condition wherever possible.
  • Develop, monitor, evaluate and review care delivery by the team, working collaboratively with the GP's and practice colleagues and other agencies.
  • To prioritise the daily workload in the absence of other staff.
  • To work in collaboration with and deputise for the district nurse as
    appropriate in line with KSF.
  • Plan, co-ordinate, evaluate and review professional activities.
  • Evaluate, prioritise and review demands for services in collaboration with team members.
  • Manage the implementation of change in work activities and services.
  • Act as an advocate for patients as required.
  • Ensure all actions are consistent with BDCFT Clinical Governance policies and procedures.
  • Enable, encourage, and support individuals, families, groups, and
    communities to address issues which affect their health and social well-being.
  • Represent individuals and families' interests when they are not able to do so themselves.
  • Identify and occasionally deliver training/talks to individuals/groups as required.
  • Ensure patients and carers have access to appropriate information,
    enabling them to make an informed choice regarding their health.
  • Frequent exposure to body fluids, blood, and wounds. Occasional
    exposure to verbal aggression.
