Job Description
Operational Delivery
- To deliver coordinated interventions within the framework of The Healthy Child Programme as delegated by the Health Visitor.
- To assess the health needs of the child and community involving the parent/carer and /or child in the process of meeting those health needs.
- To monitor the health of the child and to involve the parent/carer in promoting optimum health and development of the child.
- To be aware of the deviations from the norm in health and behaviour and to intervene to protect vulnerable children by prompt action, by referral and working with other colleagues and agencies as part of a specialist team.

Patient / Customer Care
- To assist with Child Health Clinics to monitor the growth of babies and children, advising parents/carers on aspects of parenting within his/her knowledge scope of practice, and refer to the Health Visitor with any identified issues.
- To undertake developmental assessments of children under the age of 5 Per Years as directed by the Health Visitor.
- To support families with children with SEND or complex medical needs.
To deliver health promotion activities to children, and their families. - Follow up attendance at A & E Departments or Minor Injuries Units as delegated by the Health Visitor
- To visit and assess families with children under the age of 5 Per Years who move into the county.
- To be able to assess children's health needs and signpost to local support service.

Service Development and Improvement
- To participate in audit/ research as required by the Health Visiting service and the Trust
- To maintain own level of clinical competence by reading relevant literature and taking opportunities for personal and professional development
- To participate in the identification of local health needs and the development of strategies to meet these
- To participate in risk assessment of clinical and non-clinical areas, and report accidents or near misses promptly and when required using the appropriate incident reporting tool, and cooperate with any investigation undertaken
Communication and Relationship Building
- To ensure good communication channels are maintained within the Health Visiting Team.
- To work collaboratively with other statutory, voluntary and community organisations to facilitate service provision and access for families and individuals
Information Management
- To keep accurate, comprehensive, contemporaneous records and reports
- To adhere to SystmOne electronic recording processes for data collection and collation to meet key performance targets, out coming appointments in a timely manner
- Populate own SystmOne ledger prospectively two weeks in advance for efficient allocation of work.
General
- To adhere to the NMC Code of Professional Conduct and be conversant with current professional advice and developments
- To use evidence based practice to develop and maintain a high quality standard of health visiting
- To participate in clinical supervision and reflective practice

Qualification and Training
Essential criteria
- Registered Nurse or Midwife
- Mentorship and preceptorship/ENB 998 or other relevant recognised teaching/assessment qualification or willingness to undertake
- Willingness to undertake further Trust training
Desirable criteria
- Knowledge of Systm One
Experience and Knowledge
Essential criteria
- Evidence of assessment of care needs, planning, implementation and evaluation of care plans
- Ability to recognize abuse of children/vulnerable adults and to follow safeguarding procedures
- Awareness of health needs of Black and Minority Ethnic groups
Desirable criteria
- Experience of working with other agencies
- Previous experience of working in the community with children, young people and families
- Knowledge of safeguarding children procedures and relevant legislation.
- Demonstrates knowledge of government reports relating to children and young people
- Knowledge of integrated practice, Family Centre Service and Families First
