Job Description

Detailed job description and main responsibilities
Each week there is a general respiratory clinic (usually attended by three of the respiratory consultants, a middle grade and two clinical nurse specialists), where referrals from both primary and secondary care are seen. A weekly "Rapid Access" respiratory clinic mainly to see new tertiary referrals quickly is shared between the consultants. There is a paediatric tuberculosis clinic and a weekly "Difficult Asthma" clinic conducted by a consultant and CNS, as well as a weekly nurse-led asthma clinic. Monthly multi-disciplinary non-CF bronchiectasis and ventilation (NIV and invasive) clinics are also held. All consultants cover each other for clinics when "on service" or away on annual or professional leave. The CF multi-disciplinary team are involved with a weekly cystic fibrosis clinic, weekly CF annual review clinics, CF baby clinics, CFSPID clinics and ad hoc CF reviews throughout the week. In addition, we do joint CF outreach clinics throughout the Southeast region.

At King's, children with medical respiratory problems are admitted to Toni and Guy Ward and those with surgical problems to Princess Elizabeth Ward. Children requiring high dependency or intensive care are admitted to the Thomas Cook Critical Care Centre in the Variety Children's Hospital where the respiratory service provides regular support and advice for children on invasive and non-invasive ventilation. Approximately 300 children with asthma and related wheezing illness are admitted to King's each year. Many children admitted acutely via the ED with asthma or other serious respiratory illnesses are referred to the respiratory service. We provide an acute tertiary referral service to other DGHs in our region and beyond for management of paediatric empyema, pneumothorax, necrotising pneumonia, etc. with back-up from paediatric or cardio-thoracic surgery.

The paediatric cystic fibrosis service at King's has been established for over 30 years. It is an accredited Regional Centre at the hub of a mature clinical network, which extends through South London, Kent and East Sussex. The centre cares for approximately 200 children. Joint outreach clinics are conducted in 7 paediatric units in the region with the full multi-disciplinary team. Children from anywhere in the region, however, may be reviewed at or admitted to King's between outreach clinics depending on clinical need and a number of children receive "full" CF care at King's because they live locally or have particularly severe or complex problems. Transition to the adult service is well-established and includes joint transition clinics with the adult CF multi-disciplinary team which has the great advantage of being on the same site. From its inception, we have been a member of the London CF collaborative, which has produced seminal reports on early lung function in CF infants. We are a CTAP (Clinical Trials Accelerator Platform) centre which has formed a partnership with other London CF centres for children on invasive and non-invasive ventilation.
