Job Description
The successful candidate will also complete assessments for clients and carers and provide care coordination in line with the Trust Care Programme Approach (CPA) guidance and policies. You will be allocated care coordinator/lead professional for a caseload of both organic & functional clients.
You will be expected to complete the Duty role which includes initial CMHT assessments of new referrals.
The Community Mental Health Team operates from Monday to Friday during usual office hours

Detailed job description and main responsibilities
The post holder will work within an integrated, later life multi-disciplinary team within Greater Manchester Mental Health NHS Foundation Trust. The post holder will provide assessment for service users and carers, in line with the Care Act (2014). The post holder will act as a care co-ordinator as required in line with the Trust Care Programme Approach (CPA) guidance and policies.
The post holder will work to the HCPC Standards of Proficiency for Social Workers in England.
The post holder will report to the Team Manager for day to day management. Professional social work supervision will be provided by a Senior Social Worker.
The post holder will aim at all times to promote the independence and well-being of adults with mental health needs; assist social inclusion and valued community presence and enable the widest possible expression of chosen lifestyle.

Essential criteria
- Relevant professional qualification, DipSW, BA, MA Social Work, CQSW.
Desirable criteria
- Recognised professional assessor / practice teachers qualification

Knowledge & Experience

- Experience of conducting assessments identifying health and social care needs
