Detailed job description and main responsibilities Main duties and responsibilities
  • Clinical Practice
    • To provide and manage an expert nursing service for people with specialist needs
    • To provide expert nursing advice and support to patients, their families and other healthcare professionals following diagnosis and through treatment
    • To ensure continuity of a high standard of evidence based nursing care, assessing health, health related and nursing needs of patients, their families and other carers by identifying and initiating appropriate steps for effective care. This can include
      • Managing a patient caseload
      • Ordering diagnostic tests
      • Making and receiving referrals
      • Admitting and discharging patients for specified conditions and within agreed protocols
      • Running clinics
      • Prescribing medicines and treatments
    • To accept responsibility for own patient caseload, ensuring all patients have an accurate plan of care that reflects the assessment undertaken and incorporates the issues and recommendations made ensuring clear documentation in the patient’s records/hand held records.
    • To have an excellent working knowledge of key Trust/Divisional initiatives/targets and priorities in terms of operational/strategic plan for the Myasthenia service and related initiatives
    • To contribute proactively to the nursing performance management systems in terms of both clinical audit and support for areas where remedial action is necessary
    • To be actively involved in in-patient care, coordinating and communicating with ward staff throughout neurology and other wards to ensure high quality nursing care
    • Coordinate and communicate with medical and nursing teams to organise and facilitate admission of Myasthenia patients for assessment and procedures
    • Communicate with other neurology team members to ensure effective coordination between in-patient and outpatient work on behalf of Myasthenia patients
    • Attend neurology ward rounds takings a lead role in terms of the decision making process around plans of care and discharge planning
    • Provide support for patients admitted to wards for investigation of possible Myasthenia
    • Lead the provision of expert advice and support working within existing operational policies and nurse led guidance to ensure discharge plans, care and length of stay are optimized
    • Actively contribute to the ‘back to the floor’ initiative and the development of general nursing service across the division
    • Follow-up patients as appropriate and ensure all relevant information is available to enable patients to make an informed choice about their treatment
    • Actively participate in multidisciplinary team meetings ensuring all patients are represented
    • Actively participate in the ward visual management system, ensuring the boards reflect an accurate and comprehensive plan of care for all Myasthenia patients
    • Lead the identification and development of clinical protocols and strategies to enhance both the continuity and standard of specialist care whilst ensuring equity of access to the service
    • Ensure operational policies are reviewed annually in accordance with national standards
    • Ensure all care is given in accordance with agreed protocols
    • Work with key members to establish nurse led out-patient follow-up services. Provide detailed baseline patient assessment and education and support to patients and their families
    • Regularly review those clinical notes of patients requiring imminent admission, ensuring all assessments, diagnostic investigation and test results are up-to-date and in place to ensure a smooth admission process
    • Liaise with the discharge coordinator and ward staff to ensure appropriate and timely discharge and follow-up arrangements are in place for patients
    • Identify, implement and maintain a suitable mechanism for multidisciplinary team referrals to the Clinical Nurse Specialists
    • Work with the multidisciplinary team to develop, implement and evaluate integrated care pathways and systems of MDT documentation
    • Work with local community, social services, paramedics and voluntary organisations in order to develop existing services for patients, carers and health professionals
    • Actively involve service users in providing feedback of their experience of the current service and suggestions for improvements
    • Lead on the development of appropriate patient group directives to facilitate and enhance the discharge process
    • Participate in relevant forums at Care Group, Trust, National and International Level to represent the service perspective

    • Participate in and facilitate research programs within the Myasthenia service
