Job Description

Detailed job description and main responsibilities
  • The service will provide a number of direct and indirect interventions aimed at primary care or the CYP and their families to enable the CYP's asthma or wheeze to be managed safely and effectively in the community.

To support the nursing team within the Children's Integrated Asthma and Wheeze Service to deliver treatment and intervention for children with asthma and wheeze. To work within a clinic environment, home setting, GP surgeries and school environment supporting children with the asthma and wheeze pathway set out by the nursing team.  

  • To set up clinic space ensuring all medical devices required are present and suitable interactive toys are available
  • To height and weight children upon arrival to clinic and record appropriately in the child's records.
  • To be able to be proficient in peak flow monitoring and inhaler technique.
  • To conduct inhaler re-assessments as delegated by the Asthma and wheeze nurses.
  • To participate in clinic settings by assisting in playing with the child at an age appropriate level for them to assist them understanding their needs
  • To undertake and manage routine clerical tasks in support of the integrated asthma and wheeze service.
  • To participate and support training of school staff and other professionals in inhaler's, spacer devices and how they are used.
  • Encourage and support the child and family with the asthma/wheeze pathway prescribed by the asthma and wheeze nurse.

Operational Delivery
  • The post holder will be expected to work in partnership with families, Children and Young people demonstrating sensitivity to the family's needs at all times
  • To promote and maintain a suitable environment in which the child feels safe and comfortable whilst providing any cares.
  • To ensure the safety of the child by reporting any concerns or changes in the child's health condition.
  • To ensure confidentiality is maintained at all times.
  • To gain consent and work within the legal framework with children, young people and their families.
  • Book interpreters for clinics or school/home appointments with parents who require the service.
  • To use electronic patient records accurately and in-line with the trusts IG policy.
  • Request notes, clinic letters, scans, blood test results and x-ray results when required.
  • Act as a service link to other members of the Multi-disciplinary team.
  • Be competent in using and ensure competency in the use of equipment, hoists, wheelchairs, trust equipment and the disposables that align with this.

Service Development and Improvement
  • To report clinical and non - clinical incidents in line with the Trust and departmental risk management policies.
  • To adapt and quickly respond to changes in service, patient's or departmental needs
  • To work with nurses within the community, ensuring policies and guidelines are adhered to.

Management and Leadership
  • To act upon complaints with the support of the Children's Asthma and Wheeze Nurses, complying to trust procedures.
  • To ensure good practise and professionalism is demonstrated at all times by leading by example.
  • To attend team meetings, clinical and safeguarding supervision as required
  • To ensure all trust mandatory training has been attended and to work through competencies with the support of the team
  • Maintain up to date equipment lists and timely servicing plan.

Communication and Relationship Building
  • As part of a team, deliver person centred care whilst meeting the service standards and objectives.
  • Participate in team meetings and offer suggestions for quality improvements.

Finance and Resource Management
  • Maintaining stock levels for equipment and disposables used by the Children's Asthma and Wheeze Team.
  • Ensure the equipment which is loaned is maintained and serviced in accordance with guidelines from the Medical devices Agency and the Hertfordshire Community Trust Guidelines .

Information Management
  • Maintain patient confidentiality at all times.
  • To assist the nurses in keeping the medical records of patients safe and confidential.
  • Undertake delegated clerical duties including filing, finding notes and administrative tasks, adhering to the Data Protection Act.
  • To maintain clear, concise and legible documentation, adhering to standards in accordance with the Trust policies for the purposes of communication and audit.
