Job Description
Main duties of the job
Provide a proactive role within the OPAL ED service to identify and support with the holistic assessment of frail older adults at the front door.
Manage patients on the frailty virtual ward with support of the wider OPAL MDT.
Support with clinics and the development of further ambulatory services.
Provide frailty reviews of older adults across the hospital site and support with development of our-reach services.
Support the Older Person's Assessment Unit (OAU) including attending consultant ward rounds and assisting with discharge planning and patient flow
Act as a link between the MDT, patient and their relatives / carers and utlise excellent communication skills to promote collaborative working
Deliver education and share specialized knowledge of frailty to colleagues, patients and relatives.

The Older Person's Assessment and Liaison service (OPAL) is an MDT consisting of frailty nurses, occupational therapists, physiotherapists, therapy technicians, consultant geriatricians, junior doctors and physician associates. We aim to work collaboratively to provide best care for frail older adults and complete comprehensive geriatric assessment.

Experience
Essential criteria
- Registered Nurse with 2 years experience at band 5
- Working autonomously
- Conducts research and/or clinical audit
- Ability to take a role within the multi-disciplinary team
- Knowledge of national and local targets
- Evidence of working within other relevant areas of healthcare
- Knowledge of the clinical governance agenda
Desirable criteria
- Experience working with multi-disciplinary students
- Experience in delivering teaching / training
Education/ Qualifications
Essential criteria
- Registered nurse
- Knowledge / experience of caring for frail older adults
