Job Description
Act as care co-ordinator for a designated group of patients in accordance with current and future national and local guidelines. See effective care co-ordination in mental health services: Modernising the care programme approach (NHSE 1999)
- Construct, implement and evaluate packages of care to effectively meet the needs of clients and their carers in accordance with the current guidelines for the care programme approach.
- Clinical Assessment and risk Assessment
- Undertake accurate, timely and skilled assessments and ongoing reassessment of patient's condition, taking into consideration all social, physical and environmental factors analysing a range of options
- Comprehensively and accurately record and evaluate the implications of the information gained in order to plan the appropriate care for the patient.
- Carry out accurate and in-depth assessment of risk in collaboration with other members of the multidisciplinary team. Make a competent and reliable evaluation of levels of risk presented by and to patients. Make safe and constructive decisions based on assessed risks. Record and communicate risk information to others so as to promote the safety of the patient and others.
- To undertake rostered duty role which includes assessment and triage, responding to and supporting CMHT service users as required.
- Up to date working knowledge of issues associated with dementia, undertake assessment, facilitate professional reviews of service users within our Memory Service and complete outcomes identified within these.

Qualifications
Essential criteria
- RMN with Current Registration on NMC Register.
- Full UK Driving Licence
- Slip/ student assessor
Desirable criteria
- clinical supervision training

Experience
Essential criteria
- minimum of 1 Per Years experience at Band 5 or equivalent responsibility in a mental health setting
Desirable criteria
- Experience of working with people with dementia and or their families
- Older people's experience
- Experience of supervising/ mentoring junior staff and or students
Knowledge
Essential criteria
- Current professional issues
- Relevant MHA/ MCA legislation/ guidance
Skills
