JOB PURPOSE:

The Care Coordinator oversees the management of a specific floor, department or area reporting directly to the Deputy Manager with regards to all areas of care. They will manage their area by being highly visible, involved and accessible at all times. Responsibilities include but are not limited to training Care Assistants for their department, facilitating inductions, dealing with team member performance, one-to-one meetings, bi-yearly performance reviews and ongoing coaching and counselling of team members and facilitating purposeful activities.

The responsibilities include providing tailored care, providing outreach and support to families, retaining dedicated Care Assistants, and facilitating ongoing training and educational opportunities for staff and families. The Care Coordinator is expected to be knowledgeable and constantly learning about the most current information on Dementia. This person will adhere to the dementia Strategy within their role at all times.

Care Coordinators will also help coordinate the Day Club Programme, work with families for transport, escorts to home, and facilitate person centred activities.
