Job Description

Detailed job description and main responsibilities
To lead and inspire the team to deliver a high quality, safe and effective
inpatient services for service users and carers within a defined
speciality :
Acute adult
Rehabilitation
Acute older adults functional.
Acute older adults organic

Under the direction of the Locality Clinical Lead, to take a lead role along
with the ward manager in quality and safety, through clinical governance,
adherence to best practice and audit. Providing clinical leadership and
guidance to the team ensuring that the Trusts Quality strategy is embedded within every day activity by front line staff members.

To undertake and support the specialist assessment of physiological and/or
psychological functioning for service users with complex and health and
wellbeing needs, in an emergency for service users, who may be acutely
unwell, distressed and in crisis.

This may include:
a. The use of standardised assessment tools, i.e. the Cluster Allocation
Support Tool (CAST), KGV
b. History, strengths and aspirations
c. Mental state
d. Impact of culture and diversity
e. Functional needs
f. The needs of family and carer
g. Evaluation of risk
h. Physical health
i Complicating factors
j. The interventions and treatments required to enable positive change.
k. Social care needs
l. Safeguarding and public protection
m. Capacity under the Mental Capacity Act

To be jointly responsible with the ward manager to facilitate and develop
robust joint working arrangements with the Intensive service, Recovery
service and other provider agencies. To review, develop and oversee the
delivery of comprehensive and complex discharge and home treatment
programmes using appropriate frameworks in line with evidence-based
practice, including strategies to manage risk for service users with complex
needs, utilising carers views and other resources, as required.

Alongside the Ward Manager oversee and support student
placements/experience in liaison with the Practice Education Facilitator and key mentors.

To plan, deliver and evaluate defined specialist therapeutic interventions
within sphere of ward speciality working directly with both qualified and
unqualified staff.

This might include:
a. Family intervention
b. Psychological therapies such as CBT.
c. Medication management
d. Interventions under the Mental Health Act/Mental capacity Act

To lead, in partnership with the Ward Manager and the therapies service
lead, in the setting up and delivery of a comprehensive range of defined
therapeutic activities/interventions for all inpatients including evenings and weekends.

To ensure, in partnership with the Ward Manager, the service develops and
maintains excellent clinical standards this will be underpinned by robust and effective individual and team supervision processes.

To oversee and ensure, across the team, there are effective risk management plans and strategies, for service users and carers, taking
account of risks to service users, carers, the organisation, staff and the public.

In collaboration with service users, carers and staff, to be responsible for
facilitating and ensuring, the development and implementation of complex
comprehensive care plans, advance statements etc., involving other provider agencies/ services including those from primary care,3rd sector etc where appropriate.

To lead in partnership with the Ward Manager, Medical Lead and other
allied Health Professionals. the development of a physical care training
programme. To work directly with the care team in the development of service user specific care plans for the safe management and delivery of care and treatment relating to physical problem. To organise mock scenario
programme for medical emergencies I.e. cardiac arrest.

Develop and maintain communication with a wide range of people, services, agencies about complex matters or in complex situations with the purpose of sharing developing or resolving difficult or complex issues for the benefit of service users/carer and the service.

To represent the organisation in a range of settings, presenting, facilitating
or explaining as appropriate and dealing with any questions or difficulties as they arise.

To personally build and lead others to build, hope inspiring relationships
with service users, which acknowledge the personal journey of each person, and focus on strengths and aspirations to allow the creation of meaningful
personal recovery plans.

To support the Ward Manager in developing systems and processes for
