Job Description

We are Manchester Local Care Organisation - an exciting, new public sector organisation that is bringing together NHS community health services and social care services in the city. We're driven by the aim of improving the health of local people in Manchester by working across organisational boundaries.

The first health and social care staff have been deployed to us from April 2018 and are now starting to work in new ways across the city. Our teams work as one in the neighbourhoods they serve - delivering and designing services in partnership with local people. We also provide key services that work across wider localities and city-wide. Within MFT, we are currently recruiting to permanent roles for our Integrated Community Nursing sites across our 4 patches that focus on both transitional and on-going care of patients within a community setting:

• Wythenshawe, Northenden
• Chorlton, Didsbury and Burnage
• Wythenshawe Central
• Withington, Fallowfield

The hours of the service are from 8am until 10pm over 7 days and a variety of shift patterns will be available.

This role requires the postholder to undertake duties within a care home setting. In accordance with the Coronavirus Act 2020, prior to the take up of this position. The successful applicant will be required to provide evidence they have received two doses of an authorised vaccine , or provide evidence of medical exemption in accordance with the Covid 19 Regulations. This verification will be required at or before interview / selection. Interested applicants must ensure this verification is available to be produced when requested.
