Job Description

Opal house community assessment ward, is for Elderly frail individuals whom require further rest and recovery following an acute inpatient stay and may have further social needs upon discharge that need to be assessed and facilitated.

Opal house is a short stay unit with it's own Therapy team, Medical team, Advanced Nurse Practioner and Nursing team in place to approach discharge planning and assessment of care in an MDT environment. The team is dynamic and evolving and we require keen enthusiastic team players to join us to help promote elderly care going forward to help ensure good continuity of care for the patients. All meals are cooked fresh on site by our own Chef and the patient's usually love the food on offer here making it an enjoyable social event at meal times which helps lift patients mood and engagement in activities.

There are 41 inpatient beds over two floors, there are two secure gardens also for patients, staff and visitors to use whilst staying here, the unit is secured for the safety of vulnerable patients and the use of DOLs is essential for some of our more vulnerable adults.

Monday to Friday the therapists and medics are on duty with MDT taking place every Wednesday, admissions and discharges take place daily and need to be well organised and planned to ensure safe discharges and appropriate admissions. Pharmacists visit the site daily and also we have delivery/post drop offs every weekday morning. Out of hours there is a medical bleep and also we rely on ambulance services to support urgent transfers of care for ill patients.

  • Degree or Diploma


Desirable criteria
  • Teaching Qualification
  • Degree level 6 study
  • Diploma


Experience
Essential criteria
  • Experience of working on own and as part of a team
  • Ability to supervise and support junior staff
  • Evidence of teaching and assessing skills/ability.
  • Awareness of clinical audit


Desirable criteria
  • Evidence undertaking a mentor/preceptor role
  • Experience in relevant area of practice

Trust policy requires that the cost of submitting & processing the successful applicant/s DBS application be recovered via salary deduction following start in post. The amount of £26.85(standard disclosure) or £43.85 (enhanced disclosure) which will be deducted from salary, in the first 3 months of employment. If you wish to withdraw once a DBS application has commenced you will be still liable for payment.
