Job Description

Main duties of the job

The post holder will provide advanced and expert level of practice (including non-medical prescribing) within the adult CMHT leading to improved patient outcomes.

The post holder will support the clinical and operational team to deliver on service improvements in the quality of care and safety through service development, evaluation, and audit.

They will be an integral part of the MDT supporting clinical decision making and supporting the service pathways, developing nurse led clinics and providing brief interventions. The post holder will also support the development of the Living Well Model, by providing a seamless pathway into & out of the CMHT, once interventions / goals have been achieved, to promote recovery and self-management, as clinically appropriate.

The Advanced Practitioner will hold a case load and will lead assessment, formulation and prescribing of care planning including discharge planning within agreed protocols and procedures.

The job holder may additionally lead on practice and service developments. The job holder is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice.

To support improving workforce knowledge and practice through the contribution to a CMHT education and training programme across the community teams within Bolton

To participate in the BOC/BOS on Call rota

Detailed job description and main responsibilities
To demonstrate high-level communication skills to Service Users who have complex emotional, physical and psychological conditions.

To effectively overcome the barriers to communication

To utilise highly developed interpersonal skills to facilitate effective communication when there are barriers to understanding or in a hostile, antagonistic or highly emotive situation.

To establish and maintain robust communication networks with clients, carers, team members, other health care and social care workers and external agencies.

To establish and maintain good working relationships with GPs, and other partner agencies

To represent the service at local forums.

To present at local and national forums as required

Person specification

Education/Qualifications
Essential criteria
  • Registered First level nurse MSc in Advanced Clinical Practice Nurse Prescribing qualification


Experience
Essential criteria
  • Experience of working with clients with complex needs Compiling protocols and clinical guidance Audit Experience of working at a senior clinical nurse level


Desirable criteria
  • Research


Knowledge
Essential criteria
  • Advanced Clinical Practice skills Management of service users with complex needs


Desirable criteria
  • Knowledge of issues affecting the client group


Skills and Abilities
Essential criteria
  • Experience of leading / supporting complex RCA investigations & completion of 3 day reviews & complaints Communication skills, both written and verbal Communication of difficult messages to service users and families Negotiation and conflict management skills Change management
