Job Description

Main duties of the job

The HBT operates 24/7 to provide responsive mental health care in the community setting, delivered by experienced and established multidisciplinary teams. We aim to provide an alternate to hospital admission and care for service users in the least restrictive way. As a Trust, we are committed to our services delivering models that are clinically led, operationally partnered and academically informed.

The post holder will be required to work 37.5 hours a week including weekends, bank holidays and nights. This service operates over 24 hours, 7 days a week, and so there will be times when the post holder will need to work out of hours to facilitate line management and support practitioners. There is an expectation that the post holder will participate in out-of-hours bronze on call - this is subject to approval.

Detailed job description and main responsibilities
Job Description

Provide clinical leadership, supervision, support and consultation to staff in the Home-Based Treatment Team, in line with clinical pathway as outlined within the operational procedure.

Take a lead clinical role in an identified clinical pathway for example the gatekeeping pathway

Manage and evaluate a caseload in line with relevant clinical pathway as outlined within the operational procedure and will include:
  • Assessment
  • Formulation
  • Therapeutic intervention as required
  • Signposting
  • Close working with all colleagues
  • To be responsible for a complex caseload

Act as an assessor undertaking bio-psychosocial assessments.

Take responsibility for resource allocation and lead the in-reach required for inpatient and community services.

Deputise for the Team Manager in his/her absence and take on any delegated management duties as required.

To participate in the bronze on call rota this is subject to approval.





Person Specification

Qualified RMN, OT or Social Worker

Post registration experience of working in multi-disciplinary Home Treatment team, liaison mental health or other mental health community services.

Evidence of post registration specialist training and development.

Experience of participating in audit and an understanding of implementing Action plans relating to the outcome of audits.

Knowledge of requirements under the Care Programme Approach including the needs of carers and the mental health Minimum Data Set.

  • Excellent understanding of the interface between physical and mental health needs
  • Knowledge of legislation, statutory and good practice guidance relating to mental health (including: NICE guidelines, the Mental Health Act, the Care Programme Approach and the Mental<
