Main duties of the job

- To provide comprehensive assessment of a service users mental state and social needs which would aid the completion of a collaborative care plan for community management.

- To provide comprehensive risk assessment, formulation and management as part of the service user's journey.

- To act as keyworker or co-worker for a small caseload of service users, with support from the multi-disciplinary team.

- Liaison with other agencies involved in a service users care.

- Lead on the discharge or transfer of care for our service users to appropriate agencies.

- To support and supervise junior members of staff when appropriate.

- To participate in the on call rota.

Detailed job description and main responsibilities
If you are a dynamic, motivated and forward thinking practitioner who wants to work in a busy and fast paced service within urgent care then this opportunity will be of interest to you.

You would be required to provide psychologically informed nursing interventions, engage in therapeutic risk decision making with good understanding of safeguarding who understand how psychological formulation informs the provision of care.

You would work as part of a multi disciplinary team and deliver treatment to service users and their family and carer's as a key or co-worker within a dedicated core care team (Band 6, Band 5 and support Worker).

  • Degree in Mental Health Nursing or equivalent


Desirable criteria
  • Additional qualification or training relevant to the role


Experience
Essential criteria
  • Acute or urgent care experience, either qualified or unqualified


Desirable criteria
  • Community based experience


Knowledge
Essential criteria
  • Experience and knowledge of risk assessment and care planning


Desirable criteria
  • Additional training in assessments relevant to the role


Skills

  • Ability to work under pressure in the urgent care setting
