Job overview

This is a great opportunity for a Band 5 Occupational Therapist or Nurse to develop their skills and knowledge through working in a community setting.

Southwest Kent CMHSOP specialises in the care and treatment of people over the age of 65 years who experience mental health problems and the care and treatment of people of any age who have dementia, or who have been referred for memory assessment.

We work closely with individuals and their carers and family to provide support that aids recovery or that optimises mental wellbeing.

We use a range of assessment tools to identify risks and needs and work collaboratively to develop person centred care plans to support patients.

We provide supervision and appraisal and encourage our staff to develop their knowledge and skills through both internal and external training.

Main duties of the job

Carry out initial assessments for patients who have been referred to the team.

Manage a caseload of non complex patients which involves working collaboratively to develop person centred care plans, identify risks and finding ways to reduce and manage risks. Visiting patients in their homes to monitor care and treatment.

Participating in multi-professional meetings, complex case discussions and team meetings.

Supporting carers and signposting to other agencies to provide additional support.

Manage a caseload of patients.

Person specificationQualificationsEssential criteria
  • Degree in Mental Health Nursing or equivalent Allied Health degree
  • Qualification

Desirable criteria
  • Experience of working with older people
  • Work experience

Knowledge of mental health in old ageEssential criteria
  • Knowledge of mental health issues that affect the older person
