Job Description

Main duties of the job

Be a full member of the multi-disciplinary team (MDT).

To establish and maintain good working relationships with GPs

To participate in the treatment, monitoring and review of patients with possible cognitive impairment or dementia, as defined in the NICE guidance.

To provide skilled mental health nursing care to clients.

To manage a caseload of people with organic impairment.

To assess, develop, implement and evaluate treatment with the supervision of senior team members

To act as a lead professional for non complex service users on your caseload, ensuring the needs of the service user and carers are met.

To undertake and record comprehensive risk assessments and management plans

Be expected to undertake assessments, in line with Mental Capacity Act 2007, Safeguarding Adults and Mental Health Act.

To regularly participate in Line Management and Clinical Supervision and provide this to junior members of the team.

Detailed job description and main responsibilities
Under the line management and clinical supervision from within the MATS, the post holder will:
  • Participate in the mental health nursing assessment, monitoring and review of patients with possible cognitive impairment or dementia, as defined in the NICE guidance,
  • Maintain and co-ordinate timely and appropriate interventions for an allocated caseload of service users of any age living in the community within the framework of CPA, and in line with the relevant dementia assessment clinical pathway as outlined within the MAS/CMHT operational procedure.
