Job Description

Detailed job description and main responsibilities
Response within manageable time frame and professional accountability to patient referral following jointly agreed procedures.

Confidential and comprehensive mental health nursing assessment that includes a Risk Assessment and a Care Programme within ICPA along with Risk Management Plan

To contribute to medical, nursing and multi-disciplinary care planning providing comprehensive information for determining the distribution of the Community Care Budget.

Following assessment, to refer on to other health services for further assessment in order to clarify further service requirements for individuals.

To facilitate prompt referral to other health, social and voluntary services as appropriate.

To use evidence based practices and psychosocial interventions to assess, plan, implement and evaluate nursing care.

Maintains up to date knowledge of relevant medicines, related medication management issues and acts in accordance with the "Guidelines for the administration of medicines" (UKCC 2000) and the National Institute for Clinical Excellence (NICE) guidelines for the use of anti-dementia drugs (2001).

Administers medication orally and by injection.

To advise on medication management issues.

Demonstrates a working knowledge of the key elements of the National Service Framework for Older Adults, the Community Care Act (1990), the Mental Health Act (1983).

To demonstrate knowledge about the existence of co-morbid physical pathology that may deteriorate the mental health of an older adult.

To work with up to date strategies for a range of mental health problems e.g. dementia, depression, dual diagnosis, thought disorders, drug and alcohol dependency requiring community detox programmes.

To work autonomously, using own time effectively to manage workload and the workload of others within the team.

Making use of evidence based strategies and psychosocial interventions in the care and treatment of people with mental health illness.

Enabling staff to use evidence based strategies and psychosocial interventions in the care and treatment of people with mental illness.

Act as a Clinical Supervisor and appraiser to an agreed number of staff.

To clinically manage other staff within the team.

Response within manageable time frame and professional accountability, to requests for information and advice following jointly agreed procedures.

To communicate with patients, relatives and carers.

To promote positive attitudes, mutual understanding and collaboration between hospital staff and other services involved in the care process of older adults with mental illness.

To provide specialist and effective advice and communication with primary health, nursing and residential homes, social services and other secondary mental health services following jointly agreed procedures.

To provide a source of information on how to best meet the mental health needs of adults for staff, service users and carers.

Regular provision of evidence based advice and training to staff and students including person centred approaches to dementia care (Kitwood 1990).

Formal feedback from training sessions

Evidence of development of staff competencies within the team.

Evidence of positive attitudes and evidence based interventions reflected within nursing, medical documentation.

To contribute to the provision of an appropriate learning environment for pre and post registered students. This may include formal teaching sessions. Maintaining the learning environment through best practice standards, use of research and development.

Maintaining and up dating the learning environment of the Clinical Placement area requirements. Evaluating through student feedback, faculties of health and social care input and mentor self appraisal.

Abide by the legal rules, statutory regulations and professional responsibilities relating to practice as outlined in the "Guidelines for Professional Practice" (UKCC 1996) and the "Guidelines for mental health and learning disabilities nursing" (UKCC 1998). *

Act at all times in accordance with the guidelines outlined in "Code of Professional Practice" (UKCC 1992). *
