Job Description

Main duties of the job

Part of a Comprehensive Multi-Disciplinary Mental Health team to provide appropriate timely clinical interventions to prisoners within the Health and Justice Network . In Conjunction with the management team contribute to and deliver a comprehensive stepped care model of mental health service delivery in partnership with GMMH Primary Care and key stakeholders.

Operate as part of a multi-Disciplinary team , functioning in the role of a Mental Health Nurse Practitioner to undertake MH Assessments , provide and evaluate within the service model a wide range of clinical interventions to meet clinically identified individual needs.

Display knowledge, skills and understanding of mental health issues and the treatment options available and share as appropriate with wider stakeholders , attending various multi agency meetings.

Detailed job description and main responsibilities
Inclusive varied practice as a member of the Mental Health Service team will incorporate working alongside Psychiatrists , Psychologist ,Registered Mental Health nurses , Psychological Wellbeing Practitioners , Counsellors to implement the Structured Stepped care service model of delivery . Successful candidate will engage in implementation of Stepped care model of service delivery via Mental Health triage screening assessments , Weekly team Referral meeting (SPR) , Mental health crisis role on a team rota basis in conjuncture with our custodial partners (ACCTS) , Caseload Management (CPN) role for your allocated Clients , engage in regular clinical reviews evaluating the service pathway of clinical need incorporating the Wider service MDT for positive clinical outcomes ,pre-planning and co-ordinating discharge CPA procedures /transfers and administering of medication as clinically required around the site .

Engage in regular meetings to enable and facilitate positive outcomes with our healthcare partners in primary care , pharmacy and GPs . Alongside Justice partners and the multiple departments on site such as Security , safer custody , Immigration (UKBA) ,Shelter , Chaplaincy and Probation Services .

Ensure all GMMH Mandatory Training is completed and updated as required ,Engage in Regular clinical and Managerial Supervision , complete yearly appraisals to identify any area of Clinical developmental opportunities that can be supported to enhance the role and service , personal clinical skill development is actively supported by line management

Expectations to work as part of a 7 day service with our core hours Monday to Friday ( 8-6) with occasional weekends on a staff rota basis approx. once a month

Flexible working arrangements can be discussed within the service needs being met .

Qualifications
Essential criteria
  • Post registration qualification relevant to the role
  • Understanding of MHA legislation and its application
  • Registered Mental Health Nurse Educated to degree/Diploma level or equivalent.
  • Post registration experience working within a relevant clinical setting
