Job Description

Detailed job description and main responsibilities

Act as a role model for colleagues and others in relation to professional and personal conduct and practice.

Ensure implementation of agreed standards of nursing care.

Ensure each Service User/patient has a defined individual care programme to meet their needs.

Ensure all policies and procedures relevant to the role are implemented and complied with.

Ensure that regard is given to dignity, choice, self-esteem and well-being of service users and carers.

Use nursing interventions in a variety of settings to engage with Service Users and using community resources where possible.

Ensure that agenda for social inclusion is reflected in interventions offered and implemented.

Discharge professional responsibilities under Kent & Medway NHS & Social Care Trust Child Protection and Safeguarding Adults procedures, the Mental Health Act 1983, the Mental Capacity Act 2005 and other relevant legislation.

Use the Trust's IT system as and when appropriate.

Collate evidence of own CPD and ensure junior staff do likewise through the supervision process.

Feed into the supervision process when necessary to highlight areas of poor practice and support staff to improve areas of concerns.

Continue to comply with the Nursing and Midwifery Council and ensure the Guidelines on accountability, confidentiality and administration of medication and other Guidelines that may apply from time to time are adhered to.

Responsible for a defined complex caseload within the Team. Using specialist Mental Health knowledge and skills, carry out individual nursing assessment, plan and formulate and evaluate packages of care.

Undertake the task of care co-ordinator, organise CPA review meetings, discharge planning meetings involving Service Users and their carers and other professionals/agencies, attend inpatient reviews and act as Service User advocate if necessary.

Facilitate the development and use of care pathways by the multi-disciplinary team.

Treat clients with both short term and long term conditions where assessment has indicated this. Assess and treat Service Users with complex, severe and enduring mental health conditions who may be on a community section of the Mental Health Act.

Facilitate the transfer of client from Primary Care to Secondary Care Mental Health Services as appropriate and discharge Service Users from caseload to appropriate agencies when nursing interventions are complete.

Act as a support and specialist adviser to the Band 6 and Band 5 Nurses in the team and also to GP's and Primary Care colleagues.

To be part of the Senior Team within the Locality and attend relevant regular meetings as scheduled.

Carry out agreed nursing procedures and clinical treatments in accordance with agreed policies.

To attend Tribunals and other Multi-Disciplinary Meetings and prepare reports.

To supervise and participate in the safe carriage, storage and delivery of medication to clients both in the clinical area and domestic environment, according to Trust and National Guidelines and policies, ensuring that ordering and delivery of clinical equipment meets policy requirements.

To act as mentor or delegate appropriate mentor for nurses from other clinical areas wishing to gain experience in the team as well as acting as a role model to staff within the team.

To refer as appropriate carers in need of a carers assessment in accordance with the Care Act.

  • Must be a registered Mental Health Nursing qualification
  • Minimum of 5 years relevant experience including 3 within a community setting
