Job Description

Main duties of the job

The post-holder will contribute to the development and integration of specialist nursing interventions for adults with mild to moderate cognitive impairment within the Memory Assessment Service.

The post-holder will contribute to the development and delivery of individually tailored service for people with dementia.

The post-holder will undertake assessment and treatment of clients referred for investigation of possible mild cognitive impairment and dementia, in accordance with NICE Guidelines and evidence-based practice, to support the work of the MATS service.

The post-holder will undertake supervision of junior staff and trainees, as delegated by the MATS Team Leader

The post-holder will take an active role within the relevant multi-disciplinary teams and will work within these services co-ordinated frameworks.

The post-holder will contribute to audit and research work undertaken by the MATS Team.

Detailed job description and main responsibilities
The successful applicant

Must demonstrate high-level communication skills.

Will utilise highly developed interpersonal skills including specific skills to facilitate effective communication when there are barriers to understanding e.g. sensory or cognitive impairment or in a hostile, antagonistic or highly emotive situation.

Establish and maintain robust communication networks with clients, carers, and other health care workers and numerous external agencies.

Establish and maintain good working relationships with GPs, GP commissioner's and relevant consortia.

Will promote awareness of the role of Memory Assessment Service within local service area/team, GP practices and the wider community

Undertake formal mental health nursing assessments of new clients within own caseload.

Assess, care-plan, provide interventions and evaluate and record community mental health nursing delivered to clients on own caseload.

Act as Care-Coordinator for appropriate clients under the Care Programme Approach (CPA).

Provide Specialist Mental Health Nursing Assessments, Continuing care Assessments and advise mental health nursing management plans as requested.

To promote positive mental health and physical care to clients in all situations.

To encourage and support clients to collaborate in their treatment programme and recovery.

To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients.

To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual clients and their mental health issues.

To undertake specialist mental health nursing assessments when required in line with the Mental Capacity Act 2007, The Mental Health Act and Local Authority Safeguarding Adults Procedures.

Be a full member of the multi-disciplinary team (MDT).

Effectively manage own caseload accepting referrals from agreed agencies in accordance with agreed operational procedures

To undertake and record comprehensive risk assessments, devise and implement risk management plans for clients at risk of vulnerability, self-harm or risk of physical, sexual or emotional harm to others, using the STAR Risk Assessment Tool

Ensure that service users and carers receive clear and accurate information provided

Promote a service philosophy based on a balanced model of care, recovery and personalisation.

Establish quality standards relating to the service user and carer experience

Maintain quality assurance systems to monitor and audit these standards.

To participate in complaint investigations when required
