Job Description

Main duties of the job

The Bolton division of GMMH are recruiting a registered Mental Health Nurse to join the community mental health team for older adults.

The post operates across the Bolton division and the main duties involved are those of care coordination, commissioning packages of care where appropriate, through mental health assessments, risk assessment and risk management and care planning in addition to a range of recognized mental health interventions.

The post holder will contribute to multi-disciplinary assessment processes where required and will work in partnership with the other professionals within the team and with external partner agencies.

The post holder must have access to transport to enable them to fulfill their role effectively and efficiently.

Qualifications
Essential criteria
  • Registered Mental Health Nurse


Desirable criteria
  • Post graduate qualification in specialist field


Experience
Essential criteria
  • Upto date evidence based knowledge of both functional and organic mental health disorders and assessment/ intervention tools.


Desirable criteria
  • Experience of working in a community mental health setting


Knowledge and skills
Essential criteria
  • The ability and experience of sound risk assessment skills and development of risk management plans


Skills and Experience

  • Previous work in an older adults community mental health setting


Desirable criteria
  • Experience of working in a community mental health setting


Skills and knowledge
Essential criteria
  • Knowledge of mental health conditions and how they impact on older adults


Experience

  • Experience of working within a care progreamme approach framework


Desirable criteria
  • Experience of commissioning social care services within the CPA framework
