Job Description

Main duties of the job
  • Manage a complex case load of service users under CPA
  • Attend daily zoning/ Multi disciplinary team meetings
  • Ensure all documentation is accurate and up to date
  • Provide cover for the clozapine and depot clinic
  • Carry out nursing needs assessment where required
  • Assess for commissioned packages of care where required
  • Support safe Hospital discharges
  • Liaise with other services within GMMH and external agencies
  • Provide support to Junior colleagues and wider team where required

Detailed job description and main responsibilities
The post holder will work as part of the community mental health team to ensure the mental wellbeing of service users under our care. To act as a first point of contact and coordinate the care of service users under CPA. Be able to effectively communicate with services users who are distressed and in crisis. Have an extensive knowledge base around medications and side effects monitoring. To support junior colleagues and wider team. Ensure all documentation is accurate and up to date and all KPI targets are met. Promote team moral and be a team player.
