Job Description

Main duties of the job

An excellent opportunity has arisen to recruit a fulltime CMHN in our South Manchester Later Life CMHT.

We are looking for an experienced and motivated practitioner to work with older people who are living with dementia or functional illnesses.

The successful applicant will act as care co-ordinator to service users referred to the multi-disciplinary team, assess memory and support in a range of settings in the community and 24 hour care settings. Develop care plans with service users, families/carers and other professionals ensuring needs of the service users are met.

The successful applicant will be able to demonstrate both clinical and leadership qualities whilst promoting high quality services for older people. In turn, we can offer regular managerial, clinical and professional support and the opportunity to continue to develop your practice through a range of learning and development opportunities.

Detailed job description and main responsibilities
You will work within a CMHT that includes a memory service and your role and responsibility will be diverse focusing of pateints who have been referred for a variety of mental health pronblems.

You will complete initial assessments of patients within the CMHT and develop and negotiate appropriate care plans with both service users and their carers.

You will care coordinate patients who require support within the CMHT pathway and support through their journey within the service liaising with colleagues within and outside the team.

You will complete memory assessments to aid diagnosis and support with post diagnostic follow up
