Job Description

Main duties of the job
  • To act as a Care co-ordinator within the Cpa framework thereby meeting the needs of the service user and carers
  • To organise and facilitate depot clinics/ Lithium Clinics/ Clozapine Clinics and Phit Clinics
  • Under take risk assessment and management of individuals presenting at risk of vulnerability, self harm or risk of physical, sexual , emotional harm to others.
  • To provide assessment, intervention and evaluations of packages of care for service users

Detailed job description and main responsibilities
  • Participate in the development of the integrated team and promote multidisciplinary , multi agency working,
  • To assess , develop, implement and evaluate programmes of care under supervision of the Senior Practitioner for Nursing
  • Be expected to under take assessments, in line with Mental Capacity Act 2007, Safeguarding Adults and consideration for need for Mental Health Act Assessments
  • Complete Mental Health Nursing Assessments as requested ie: Local Authority Panel Requests, Salford PCT continuing Care Assessments
  • To provide Written and Verbal reports when and as needed
  • Establish and maintain effective working relationships with statutory and non statutory organisations to maximise resources in the community
  • Participate in the role of mentor/ supervisor of health care professionals and students

Person specification

Evidence of continuous professional developement
Essential criteria
  • Demonstrates evidence in continous professional development.


Desirable criteria
  • Has completed student supervisor / assessor training


Registered Mental Health Nurse
Essential criteria
  • Qualified Mental health Nurse/ Due to qualify


Desirable criteria
  • Additional Qualifications which will support staff in the Band 5 role


Venepuncture trained
Essential criteria
  • Clear interest in managing clinics at prescott house


Desirable criteria
  • Completion of venepuncture training


Experience and knowledge
Essential criteria
  • Previous experience in working in mental health setting


Desirable criteria
  • Previous experience working in Community Mental health Team
