Job Description

Main duties of the job

The primary function of the Day Service, is to provide specialist assessment and care to adolescents who exhibit signs and symptoms of mental health problems and/or trauma associated with past abusive experiences. We also provide intervention for those with anxiety disorder, where school refusal is prevalent. The primary function of the Day Service is to reintegrate young people back into the community safely, and to bridge the gap from inpatient services where discharge into the community has proved difficult due to a slower pace of recovery whilst in Tier 4 services, however the Young Persons time within Tier 4 may become detrimental to their mental health rehabilitation

Detailed job description and main responsibilities
The Mental Health Practitioner will be responsible for implementing the Care Programme Approach. The post holder is responsible for the assessment, planning, implementation, evaluation and risk assessment and management for an identified client group.

You will liaise and meet with CAMHS teams and external agencies and offer support to the families of the young people under the care of the service.

The model is for young people to work towards their goals and discharge with in a six week period. You may be involved in co-ordinating and supporting reintegration into mainstream and alternative education provisions. At the point of discharge the Band 6 may then offer outpatient support for further step down from the service.

The successful applicant will also be involved in co facilitating groups run by the Day Service.
