Job Description

Nurses with an interest in Diabetes

The team at MySugarWatch® are on a mission to put Type 2 Diabetes into remission through needle-free continuous glucose monitoring. To achieve this, we are looking for nurses to join their team who share in their mission to change the future of diabetes and ultimately enable patients to have the freedom to live their lives.

My Sugar Watch:

Over the past 11 years Dr Chowdhury and Dr Burston have been collaborating to create MySugarWatch® with the vision to change the lives of those living with Type 2 Diabetes. The result of this collaboration is a daily Sensor which sits comfortably on (not under) the skin, monitoring glucose levels in real time without repeated scanning, and avoiding time consuming, repeated fingerprick tests at work.

Unlike uncomfortable blood glucose fingerprick tests, which show glucose levels at specific times of the day, MySugarWatch® updates readings every five minutes; tracking highs and lows, identifying trends and monitoring Time in Range (TIR).

With continuous data their mission is to enable patients to review and reflect on the impact of their lifestyle choices. That means they can adjust their diet and activity and change their lifestyle to suit.

For those already using the MySugarWatch® they are seeing the positive impact on managing their diabetes.

Role Overview:

As MSW embark on the next stage of their journey, they believe that to provide the holistic support to their members they need a team of nurses who will work with each member. They will coach and educate their members to understand their data, along with the lifestyle steps and adjustments they can make to help manage their diabetes.

They are building a team of nurses who are passionate about shaping the future of healthcare care. Finding the best nurses who are committed to supporting members to manage their Diabetes is at the core of MySugarWatch®.

You will provide one:one lifestyle, coaching, education and support to help members better manage their Diabetes based on their personal MySugarWatch® data.

Responsibilities - these will include:

• Providing effective and engaging on-boarding sessions with each member in your portfolio to enable them to understand how to use MySugarWatch®.

• Coordinate and manage ongoing progress reviews with each member through virtual consultations [dedicated member time with protected prep and admin time in addition].

• Provide education and lifestyle programmes and guidance for membersp based on the data insights.

• Provide a high quality, efficient and effective service to all members to enable them to realise the full benefits from the MySugarWatch®.

• To practice within own sphere of competence and agreed acceptable limits of practice. Ensuring the relevant sign posting to their doctor for any clinical matters related to diabetes treatment.

• To work as part of the nursing team in providing quality, data driven support to meet the holistic needs of the member.

• To take an active role to ensure service quality in line with MySugarWatch® goals.

• To assist in the ongoing development of new models for service delivery which have a positive impact on members.

Qualifications & Experience

• Registered General Nurse

• Demonstratable experience of supporting patients to manage diabetes

• An interest in developing knowledge of diabetes. Funding towards, or time off for study will be supported.

• Good organisational and communication skills

• The ability to both work under your own initiative and follow directions

Our ideal candidate will be forward-thinking, motivated, and passionate about working with people to provide personalised care. You will be an advocate for healthy living. The goal of MySugarWatch® is to enable members to access education, information, advice and support to help them to make informed choices about how to manage Diabetes [alongside their medical treatment plan]. You will support them by joint planning and goal setting. In doing so you will empower members to self-manage their health and wellbeing for the long-term.

Working hours:

We welcome applicants who are looking for either full or part time hours.

Monday to Friday (shifts between 8am and 8pm)

Some Saturday mornings (9am to 12noon)

Flexible options may be available e.g. earlies / lates /long days
